This module seeks to develop practical skills that will help you to initiate conversations with patients at times that are easier for you as well as the patient. Please remember that advance care planning skills need practice just like any other technical skill you have become so proficient in. Practicing these skills with a colleague may be a good place to start.
At the completion of this module the learner will be able to:
- Identify ESRD patients at risk to die in the next 6 -12 months.
- List 4 core skills for initiating advance care planning discussions.
- Provide 5 examples of how to implement advanced care planning.
