James E. Lett, MD, CMD is certified by the American Board of Family Practice with a Certificate in Added Qualifications in Geriatrics and is a Certified Medical Director.
Dr. Lett is a past president the American Medical Directors Association (AMDA), and has held positions and memberships in multiple medical organizations. He served on a CMS to revise Care Transitions measures for QIOs, on the AMA PCPI workgroup to develop Care Transitions measures and chaired a national workgroup to create a Clinical Practice Guideline for Care Transitions in the Long-Term Care Continuum. A member of the Advisory Board for the National Transitions of Care Coalition, he also serves on their Measures and Standards Workgroup. He was Senior Medical Director for Lumetra, the Quality Improvement Organization for California 2006-2008. While there he was the liaison to NQF and served on the QMRI Council. Dr. Lett is chair of AMDA's newly formed Transitions of Care Ad-hock committee, under the domain of the Clinical Practice Committee.
