The Veterans Health Administration PCMH Model: An Ambulatory Care Coordination PACT: This session explores the Patient-Centered Medical Home Model used by the Veterans Health Systems. Multiple metrics of the system, known as the Patient Aligned Care Team (PACT), are discussed and the collaboration of RN Care Managers and Care Coordinators as leaders of the team will be addressed to facilitate ideas on the improvement of patient care.

Integrating Population Health Management into the Patient- Centered Medical Home: Meeting MCQA PCMH Recognition Requirements: Managing the health of our population requires a focus on the health of each individual patient. This session explores how using a team-based approach in a medical home environment can lead to better outcomes for patients.
