This session shows how the Ambulatory Care Service Delivery Model is focused on service excellence, with the patient at the center. Four pillars of care delineate the four phases of the patient's experience. Standards of practice for each of the four pillars, expected outcomes, and quality management indicators are described.

  • Describe the "Ambulatory Care Service Delivery Model" and understand how it was developed.
  • Outline the process for implementing this model in a large academic center.
