The Patient-Centered Medical Home is an approach to providing comprehensive primary care that facilitates partnerships between individual patients and their providers. The goal of medical homes is to provide better access to health care, increase satisfaction with care, and improve overall health of individuals, families, and communities. This session describes this plan of care and provides guidance and education on how to implement processes that may work to meet its standards.
